Contact your personal agent (PA) to notify them of the report.

Visit the Oregon DHS website to learn more about How to Report Abuse and Neglect.

What is mandatory abuse reporting?

Oregon state law mandates that employees in certain professions, such as personal agents (PA) and personal support workers (PSW), must report abuse and neglect if they have a reasonable cause to suspect an individual is being abused. These people who are obligated to make reports are called mandatory abuse reporters and are essential to the process of protecting Oregon's most vulnerable citizens.

Who is a Mandatory Abuse Reporter?

It is the responsibility of every citizen to protect those people who cannot protect themselves. Members of the general public may choose to report suspected abuse and neglect. But employees designated as mandatory abuse reporters are required by law to make reports when they have a reasonable cause to suspect the abuse, neglect, and financial exploitation of an individual.

  • All employees of Oregon's Department of Human Services are mandatory abuse reporters.

  • All employees of Community Pathways, Inc. are mandatory abuse reporters and are required to report abuse and neglect to Oregon's DHS.

  • All providers (PSWs and agencies) are mandatory abuse reporters and are required to report abuse and neglect to Oregon's DHS.

If you are experiencing abuse or neglect, or know someone who is, please contact a personal agent and call the DHS support line 1-855-503-SAFE.
