Our New Website Launches Today

Today is the day! After months of planning, gathering feedback, and creating and editing (and re-editing) content, Community Pathways is launching its new-and-improved website!

We are excited to be sharing more information about our team, the brokerage process, community-based resources, and more with you all. This site has been created with the individuals we serve, their family members, providers (PSWs and agencies), and fellow community members in mind. We hope you find the site useful!

We invite you to connect with us to share your stories and feedback, and to let us know what other information and resources you'd like to see on our site. We value your feedback. It helps us to improve our services and further our mission of assisting people with disabilities to live empowered, self-determined lives.

Touring the Seven Corners Community Collaborative

It's Thursday, so here's a throw back to last month when members of our staff toured the Seven Corners Community Collaborative, the new building in SE Portland that CPI is moving to in June!

Community Pathways will be sharing the building with our partners: Community Vision, FACT Oregon, Oregon Council on Developmental Disabilities, and the Assistive Technology Lab. Nossa Familia Coffee will have a shop on the ground floor, which everyone is very happy about. Our team is excited to be a part of this collaborative community space where individuals and their families can seek a variety of supports and services, all in one stop.

Spreading Holiday Cheer

Happy Holidays, Everyone! Today, Santa stopped by the office to spread some holiday cheer with this plate of beautiful cookies. Yum, yum, yum. We hope everyone is having a happy and healthy holiday season.

Customers and Personal Support Workers: We want to hear from you!

Did you receive an email or letter in the mail from Community Pathways with a Feedback Survey? We've been getting many of your responses, and hope to get even more!

We want to know how things are going with the supports and services you are receiving through Community Pathways. We also want feedback from Providers so that we can help improve how we all provide support to the individuals we serve.

  • If you did NOT receive a survey in the mail or email, let us know!
  • We may need to update your email or mailing address in our system.
  • We will send you a new survey to the correct address.

Our team at Community Pathways wants your feedback because we deeply care about the individuals and families we serve and appreciate the support Providers deliver on a daily basis to assist individuals living and working in the community.
