Who does Community Pathways serve?
Welcome! If you are an existing or potential customer of Community Pathways you’re in the right place.
Community Pathways provides case management services to adults who experience an intellectual and/or developmental disability and live in their own home or family home. We serve over 500 individuals living in Clackamas, Multnomah, and Washington counties in Oregon.
Our personal agents (a type of case manager and service coordinator) help the individuals we serve to access, plan, and monitor the community-based supports and services they want and need to live in the community and work toward achieving their goals.
